When this form is opened with Adobe Acrobat, you have the option to enter
your details on screen and then print the completed form.
There are fields for your Name and address etc. plus fields for the titles you
enter in each section on the form.
To use this facility, make sure that the "Hand" tool is selected in Acrobat
and move it over the line to the right of "Family Name". The cursor should
change from the "Hand" to an "I Beam” and you can click to make a flashing
"Insertion Point" cursor visible in the Family Name field. Now you can type
in your name etc.
You can use the TAB key or click with the mouse to move from field to field.
Once the form is complete you can print it.
Please print in landscape mode on A4 or foolscap paper.
